REQUIRED FORMS

Below are the required forms to properly register your child/ren at Benton District 47. 

All forms in the Registration Packet must be completed for ALL new students entering Kindergarten through 8th grade. The Registration Packet includes:

• Notice of Activity Fee

• Student Registration Form

• Data Collection Form

• Acceptable Computer Use/Internet Safety Policy and Contract for Students

• Home Language Survey

• Emergency Medical Treatment and Parental Waiver

• Bus Registration

Emergency Treatment Form

The Emergency Treatment Form is required for ALL students attending District 47 schools.

Proof of Residency

Proof of Residency is required for ALL students attending District 47 schools.

Permission to Publish

Permission to Publish is required for students enrolling in District 47 schools.

 
ADDITIONAL FORMS

Student Accident Excess Coverage

District 47 is an eligible member of the Illinois School District Agency ("ISDAn). Student Accident Excess Coverage will be provided for the school year for each enrolled K-12 student at no cost to you. No enrollment is necessary for the school time Student Accident Coverage. Additional information regarding the Student Accident Coverage can be obtained by requesting a brochure from the District Office.

Permission for Medication

The Permission for Medication form only applies to students requiring medication to be taken during the school day.  If your child does NOT take medication at school, DO NOT complete this form.
